Home Health Nurses

In many ways, the nurse is the center of your home health medical team. He or she will coordinate the plan of care your doctor has prescribed for you with other members of your team to help meet your needs and that get you the best care possible.

·        treating and helping to prevent illnesses like pneumonia
Your nurse can also help you with issues such as pain management and will report back to your doctor if you need more help. If you have a chronic condition, your at-home nurse can teach you how to self-manage it. Increasing your independence is the goal. Of course, the specific duties of your nurse depend on your individual situation.
