Amedisys believes in the power of technology to enable our clinicians to provide better care for our patients. To truly establish a continuum of care that connects each member of a patient’s care team, technology is required to foster communication, management of care plans and documentation management. That’s why we’ve invested in programs and technologies that enable our patients to have a true continuum of care experience.

At Amedisys, we leverage technology to facilitate a higher level of care quality and coordination. Here are some examples of the tools we have deployed:
MercuryDoc
Our web-based patient management portal for physicians enables real-time communication and collaboration between you and our clinicians regarding your patient’s Plan of Care. MercuryDoc can improve the efficiency for physicians by:
·        providing secure access to your patients’ records, including graphed trends and vital signs
·        helping physicians manage Plans of Care (485s), supplemental orders and patient referrals
·        tracking and reporting time spent on Care Plan Oversight
 
Point-of-Care
Our point-of-care laptops empower our clinicians to document and access patient information and our evidence-based clinical protocols for every patient visit. This technology, paired with MercuryDoc, can provide near real-time updates that help coordinate high-quality home health care.
Telehealth
Telehealth monitoring increases the frequency and consistency of measurements of a patient's condition, enhancing the health care providers' ability to recognize early warning signs and make rapid decisions about the patients' Plan of Care. Amedisys currently offers telehealth monitoring at more than 200 locations across the nation.
Our telemonitoring capabilities include remote monitoring of vital signs (BP, HR, O2 Saturation, Blood Sugar and Weight), symptoms and compliance. We can customize each unit to focus on key triggers for each patient and the monitors are able to communicate in multiple languages.
We highly recommend use of telemonitoring for all cardiac and respiratory patients as we have noted a marked reduction in re-hospitalizations when telemonitoring is incorporated into a high-risk patient’s Plan of Care. Telemonitoring units and the accompanying care are generally included, at no additional cost to the patient, in episodic plans of treatment.
AMS
Amedisys has a nationwide, proprietary EMR system called “AMS” (an Advanced Medical System) which is automated from central intake to scheduling, medical supplies, clinical management, billing, payroll, physician communications and clinician documentation.
Amedisys has the industry's first-ever nationwide Care Transitions program, designed to reduce unnecessary hospital readmissions through patient and caregiver health coaching and care coordination, which starts in the hospital and continues throughout completion of the patient's home health plan of care.
The Amedisys Care Transitions initiative was designed based on work by Eric Coleman, MD, MPH, University of Colorado at Denver Health Sciences Center, and Mary Naylor, PhD, FAAN, RN, University of Pennsylvania School of Nursing.
